Industrial Attachment Procedures, 2015

All first and second year students are to take note of the Industrial attachment procedures:

Step 1:
Students are to submit their Acceptance letters from industry/organization to the Industrial Liaison Office.

Step 2
Students are to purchase their log books at the Bookshop (Administration block).

Step 3
In the log book are two forms: Assumption of duty form and Students’ assessment form. Alternatively you can download a fillable vision of the Assumption of duty here

Step 4
On assumption of duty on July 13, 2015, the Assumption of Duty form should be filled and send by email to this ilo@koforiduapoly.edu.gh by July 20, 2015.

Step 5
Lecturers/Supervisors will be sent to supervise all students on attachment at their various locations on a scheduled date.
